INSTRUCTIONS FOR FILING A COMPLAINT OF DISCRIMINATION

1. Fill in all information concerning your name, address, and telephone number.

2. Circle the reason for the discrimination as you believe it to exist, that is, race, color, etc.

3. Set forth a complete statement showing the details of the incident upon which you are basing your charges, include dates, names, including the names of any persons who witnessed the incident. If you need additional space, attach an additional sheet to the complain form.

4. Sign the form, date your signature, and have your signature notarized.

5. File this form with the Secretary of the Human Rights Committee.

6. You may contact any member of the Human Rights Committee for assistance in preparation of this complaint.

7. No fee is required to file a complaint.
